We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team in a skilled nursing facility. The Physical Therapist will evaluate residents and develop individualized treatment plans to improve mobility, strength, and overall functional independence while enhancing quality of life.

Key Responsibilities:
  • Evaluate residents and develop individualized physical therapy treatment plans
  • Provide therapeutic exercises, manual therapy, and mobility training
  • Monitor and document resident progress and adjust care plans as needed
  • Collaborate with interdisciplinary team members to ensure comprehensive care
  • Educate residents and families on treatment plans and at-home exercises
  • Ensure compliance with all state, federal, and facility regulations
  • Maintain accurate and timely documentation in accordance with facility policies
  • Participate in care plan meetings and team discussions
Qualifications:
  • Licensed Physical Therapist in the state of [State]
  • Graduate of an accredited Physical Therapy program
  • Previous experience in a skilled nursing facility or rehab setting preferred
  • Strong clinical, communication, and documentation skills
  • Ability to work independently and as part of a team
  • Compassionate and patient-focused approach to care
