Health Advocates Network is currently seeking a  
** Social Worker to work at a facility in Coxsackie, NY.
** These are registry positions with our company.

** Pay Rate: $50  / hour*
* ** Shift details:*
* +  
** 7am - 4pm Mon-Fri*
* +  
** Approx 40 hours/week(M-F, no weekends) *
* General duties:

Would mainly be working weekdays and assisting in providing therapeutic services to patients in a primary therapist role to include individual therapy and the completion of mental health assessments and documentation (Core History, Suicide Assessments/risk assessments, Weekly/Monthly Progress Notes, Treatment Plans, Discharge Plans) According to policy and Procedure.
