Position Overview:

We are seeking a Licensed Mental Health Professional to join a Youth Assertive Community Treatment (ACT) program serving individuals ages 10-21. This role focuses on providing family-centered, community-based mental health services to support youth in achieving stability and success at home, school, and within their communities.

Key Responsibilities:

• Conduct child and family-centered assessments and develop individualized service plans
• Provide individual and family therapy in home and community settings
• Deliver skill-building and stabilization support for youth and families
• Utilize evidence-based and trauma-informed care approaches
• Collaborate with a multidisciplinary care team

Qualifications:

• Master's degree in Social Work or related mental health field
• Active NY State license: LMSW, LCSW, LMHC, LMFT, LCAT, or equivalent
• Minimum 2+ years of relevant experience required
• Strong knowledge of evidence-based practices and trauma-informed care
• Valid driver's license required
