Elderwood at Hamburg is hiring Registered Nurses for full-time, part-time, and per diem opportunities. Our RNs care for residents with post-acute, higher-acuity, and complex medical needs while working closely with an interdisciplinary clinical team.

On our subacute rehabilitation unit, you’ll strengthen your assessment, clinical decision-making, and care-coordination skills as residents recover from illness, injury, surgery, or hospitalization. Across our care settings, you’ll also have the opportunity to know your residents over time and see the impact of consistent, attentive nursing care.

• Full-Time RN: Regularly scheduled facility position with benefits eligibility
• Part-Time RN: Consistent reduced schedule with benefits eligibility
• Per Diem RN through Elderwood Staffing Solutions: Flexible scheduling and premium per diem pay through Elderwood’s internal staffing team

Available shifts, scheduling options, and shift differentials will be reviewed during the interview process.

• Care for residents with post-acute and complex medical needs.
• Apply advanced assessment, clinical judgment, and care-coordination skills.
• Collaborate with physicians, rehabilitation therapists, respiratory services, pharmacy, and other interdisciplinary partners.
• Build meaningful relationships with residents while supporting their health, recovery, comfort, and independence.
• Work with supportive nursing leadership led by an experienced Director of Nursing.
• Pursue future opportunities in nursing leadership, education, quality, and specialty practice.

SLP:
Speech-Language Pathologist:

• Evaluate residents and establish individualized treatment plans
• Provide speech, language, cognitive, voice, and swallowing therapy services
• Complete accurate and timely clinical documentation
• Collaborate with interdisciplinary care teams
• Educate residents, families, and caregivers
• Participate in care planning and discharge planning activities
• Ensure compliance with all regulatory and professional practice standards
• Support positive clinical outcomes and resident satisfaction

SLP:
Speech-Language Pathologist:

• M.A. or M.S. Degree in Speech-Language Pathology from an approved school required.
• State license to practice as a Speech-Language Pathologist required.
• Clinical Competence Certification from the American Speech and Hearing Association preferred.
• Ability to communicate well verbally and in writing required.
• Current, valid CPR/BLS credential.
• Qualities of maturity, diplomacy, and ability to work well with others required.
• Verification of qualifications by the State Department of Health
• Rehabilitation Therapies (license number/certification number) required.
• Ability to perform tasks to established standards of excellence required.
