Our client is seeking a dedicated and highly skilled Clinical Pharmacist to join their dynamic healthcare team in Saranac Lake, NY. This full-time hospital-based role is essential for providing high-quality, compassionate care through the expert management of medication therapies. The successful candidate will work collaboratively with physicians, nurses, and other healthcare professionals to optimize patient outcomes, ensuring that every patient receives the correct medications and understands their proper use.

Key Responsibilities

• Prepare and dispense prescription medications and pharmaceutical supplies with precision, ensuring adherence to department policies and safety standards.
• Provide expert clinical consultation on drug therapy, collaborating with medical staff to adjust treatments for maximum efficacy, safety, and cost-effectiveness.
• Educate patients and their families regarding medications, including administration, potential side effects, and the importance of adherence to prescribed regimens.
• Monitor patient health and profiles to assess for adverse drug reactions and potential interactions between medications.

Qualifications Required

• Education:
  
  Bachelor of Pharmacy required;
  Doctorate of Pharmacy (Pharm.D.) strongly preferred.
• Licensure:
  Active, valid Pharmacist license in the state of New York (NY).
• Experience:
  
  2+ years of clinical pharmacy experience in an inpatient hospital setting.
• Skills:
  
  Strong communication and interpersonal skills, proficiency in hospital computer systems, and a commitment to patient-centric care.
