Job Overview - Care Manager

Compensation: $28/hour

Schedule:

Full-time, Monday-Friday 9:00 AM-5:00 PM

TAG Med Staffing is hiring a Care Manager in Brooklyn, NY to support care coordination, case management, and client services within a community-based healthcare environment. This role will coordinate services, connect clients with healthcare and community resources, collaborate with interdisciplinary teams, and advocate for individuals and families to help ensure access to high-quality, person-centered care. The ideal candidate will have experience in care management, case management, healthcare, social services, or human services, along with strong communication, organizational, and relationship-building skills.

Bilingual Spanish is preferred but not required.

Responsibilities
  • Care Coordination: Manage a caseload while coordinating healthcare, behavioral health, educational, and community-based services to support positive client outcomes.
  • Client Assessment: Complete comprehensive assessments, care plans, required documentation, and ongoing evaluations to ensure services align with client needs.
  • Family Support: Build strong relationships with children, families, caregivers, and multidisciplinary teams while providing advocacy, education, and resource coordination.
  • Case Management: Monitor client progress, facilitate referrals, coordinate transitions of care, and ensure timely follow-up with service providers.
  • Documentation & Compliance: Maintain accurate case records, complete required documentation, and ensure compliance with organizational and regulatory standards.
  • Community Engagement: Conduct community and field visits as needed while collaborating with external agencies and providers to support continuity of care.
Qualifications
  • Education: Bachelor’s degree in Human Services, Social Work, Psychology, Public Health, Healthcare, or a related field is required.
  • Experience: Experience in care management, case management, social services, behavioral health, child welfare, healthcare, or a related human services setting is preferred.
  • Industry Knowledge: Knowledge of care coordination, community resources, and health or human services is preferred.

    Skills:

    Experience with electronic documentation systems, case management software, and Microsoft Office Suite is preferred.
  • Skills & Attributes: Strong communication, organizational, relationship-building, advocacy, and time management skills with the ability to manage a caseload, collaborate across multidisciplinary teams, travel locally, and maintain a flexible schedule are required. Bilingual English/Spanish skills are preferred.
