The Child Center of NY is seeking a Resource Coordinator (Care Manager) to join our NYC program and oversee intake, enrollment and service coordination across medical, social, and behavioral health needs.
Home visits are conducted in Queens or Long Island and you will collaborate with an interdisciplinary team to ensure access to supports, advocate for families, and monitor compliance with regulations.
