Homeside Rehab is seeking compassionate and dedicated Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to provide individualized, evidence-based physical therapy services to patients in their homes throughout New York City and Yonkers, NY
.

• Perform comprehensive physical therapy evaluations (PTs) and develop individualized treatment plans based on each patient's needs and goals.

• Provide evidence-based physical therapy interventions to improve strength, balance, coordination, mobility, gait, endurance, and functional independence.

• Deliver skilled therapy services in patients' homes while promoting improved function and quality of life.

• Complete accurate, timely, and compliant electronic documentation.

• Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care.

• Educate patients and caregivers on home exercise programs, mobility strategies, and therapeutic techniques.

• Communicate effectively with office staff regarding scheduling, patient updates, and clinical needs.

• Active New York State Physical Therapist (PT) or Physical Therapist Assistant (PTA) License (Required)

• Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or Skilled Nursing Facility (SNF) preferred

• Excellent clinical assessment and communication skills

• Ability to work independently in a home health setting

• Basic computer proficiency for electronic documentation

• Valid New York State driver's license and reliable transportation

• Job Types: Full-time, Part-time, Per Diem

• Compensation: $75.00–$90.00 per hour

• Choose your own hours

• Monday through Friday

• Day and evening shifts available

• Weekend availability optional
