About the Role:

As a Social Worker, you will be responsible for:

• Assessing the social, emotional, and psychological needs of our residents and identifying opportunities for improvement
• Developing and implementing individualized care plans to ensure our residents receive comprehensive care
• Providing counseling, guidance, and support to residents and their families
• Collaborating with other healthcare professionals to ensure a coordinated approach to care
• Identifying and addressing social determinants of health that impact our residents' overall well-being

Responsibilities:

• Conduct initial and ongoing assessments of residents' social, emotional, and psychological needs
• Develop and implement individualized care plans in collaboration with the interdisciplinary care team
• Provide counseling, guidance, and support to residents and their families
• Identify and address social determinants of health that impact our residents' overall well-being
• Participate in family and resident meetings, as needed
• Maintain accurate and detailed records of resident interactions and care plans
