Middletown Park Rehabilitation & Healthcare is seeking a Physical Therapist, PT for a full-time or part-time position. Full-time temporary positions are also available for experienced candidates.

Job DescriptionThe Physical Therapist, PT is to assist in planning, organizing, developing, and implementing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by the Director of Rehabilitation and/or the Administrator, to assure that the highest degree of quality resident care can be maintained at all times.
- Physical Therapist must possess the ability to plan, develop, organize, evaluate, and implement the programs, goals, objectives, policies, and procedures, etc., of the physical therapy program to help residents obtain/maintain their highest level of physical, mental, and social well-being.
- Physical Therapist will interpret and/or instruct in physical therapy policies and procedure to personnel, residents, family members, etc. as necessary.
- Physical Therapist will participate in discharge planning, development and implementation of resident care plans, resident assessments, etc. as necessary.
- Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act.
- Maintain treatment records, resident files, and progress notes as required and maintain an adequate liaison with families and residents.
- Must possess a current license and registration to practice as a Physical Therapist, PT in the State of NY.
- Must possess current malpractice insurance, license in good standing.
- Dedicated to providing extraordinary compassionate care as a Physical Therapist, PT.
- Energetic, enthusiastic, committed, and passionate about quality care.
- Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents.
- Patience, a cheerful disposition, and willingness to assist any residents and co-workers.
- Excellent communication, customer service, and organization skills.
- Trusted to work independently and in a team environment.
- Able to work well under pressure, flexible, and will work harmoniously with all staff.
