About This Role
The Metro Plus Case Manager II coordinates and monitors patient-centered quality care, ensuring optimal resource utilization and compliance with regulations. This role is ideal for experienced nurses looking to make a significant impact on patient outcomes.
Highlights- Pay: $123,588 - $137,320/year
- Location: New York, NY
- Facility: Metro Plus Health
- Department/Unit: CASE MANAGEMENT
- Schedule: Full-Time, 9:00 A.M – 5:00 P.M
- Review each patient's chart to ensure documentation supports the plan of care.
- Coordinate and facilitate timely implementation of discharge plans.
- Participate in multidisciplinary rounds to discuss care plans and resource utilization.
- Collaborate with healthcare professionals to identify and eliminate barriers to care.
- Communicate with reimbursement departments to obtain authorizations.
- Plan strategies to reduce length of stay and improve patient outcomes.
- Initiate discharge planning by assessing patient and family needs.
- Maintain effective communication with all stakeholders involved in discharge planning.
- Engage with community organizations to arrange housing and follow-up care.
- Supervise and mentor new care management staff.
- Education: Baccalaureate degree in Nursing or related health care field required.
- Experience: 2 years of experience as a Registered Professional Nurse required.

Valid New York State license and current registration to practice as a Registered Professional Nurse.
