We are seeking a compassionate and dedicated Home Health Aide (HHA) to join our team at EOM Home Care in Far Rockaway, NY. As a valued member of our team, you will provide exceptional care and support to our clients in the comfort of their own homes.

Responsibilities:

• Provide personal care and support to clients, including bathing, dressing, and grooming

• Assist with daily living activities, such as meal preparation, medication management, and light housekeeping

• Monitor and report client conditions to healthcare teams and families

• Collaborate with healthcare professionals to develop and implement individualized care plans

• Maintain accurate records and documentation of client care

Requirements:

• Home Health Aide (HHA) or Personal Care Aide (PCA) Certificate

• Recent Physical

• Rubella and Rubeola Immunizations

• TB Screening or QuantiFERON

• Two forms of identification
