2 POSITIONS available - Schedule Sun-Thursday 2pm
-10:30pm and Tuesday-Saturday 2pm-10:30pm

FUNCTION:
To provide comprehensive clinical case management services that include: assessments; individual, group and family treatment; treatment plan coordination; referent relations; discharge planning; and intervention with client support systems, including social service programs and managed care.

ESSENTIAL DUTIES & RESPONSIBILITIES include the following. Other duties may be assigned.

• Coordination of census.
• Coordination of patient transfers.
• Overseeing the caseloads and scheduling of the Adult Program Clinical Case Managers and attend team meetings and audit paperwork as needed.

ASSIST PROGRAM DIRECTOR WITH:

• Overseeing treatment planning.
• Evaluation and delegation of rules for the Program as put forth by management.
• Addressing all conflict issues with both staff and patients.
• Conducting wing meetings.
• Reviewing and modifying the program’s services.
• Public relations with other treatment modalities to assess appropriate referral for adult clientele.
• Maintaining Coordinator’s own reduced case load and responsibilities as outlined in Case Manager (CASAC/LMSW) job description relative to licensure and title.
• Participates in Performance Improvement Initiatives.
