Mount Sinai - 1468 Madison Avenue - Responsibilities:
Schedule and navigate direct access colonoscopies for patients in The CHOICE Program;
Contact patients to schedule colonoscopy procedures and provide instructions for preparation;
Mentor less experienced Patient Navigators and assist in performance appraisal processes;
Assist with insurance verification and related matters;
Track all encounters in the CHOICE tracking database Compass Rose;
Support scheduling and navigating GI Fellows Clinic patients, in person and remotely;
Participate in data projects to advance program goals;
Provide ongoing navigation support including reminders, calls, texts, and policy reviews
