Make a Difference in Behavioral Health

We are seeking a compassionate, bilingual
Licensed Master Social Worker (LMSW)to join an integrated behavioral health team supporting patients experiencing anxiety, depression, and other common mental health conditions. This is an excellent opportunity for a clinician who is passionate about collaborative care and wants to work alongside primary care providers, psychiatrists, and behavioral health professionals to improve patient outcomes.

This is a
grant-funded, full-time position
based in the Bronx. Clinicians will work onsite at one of several outpatient locations within the borough.

Responsibilities

  • Perform psychosocial evaluations through clinical interviews and standardized screening tools.
  • Assess emotional, behavioral, and social needs while identifying barriers to care.
  • Develop individualized, patient-centered treatment plans based on clinical findings and patient goals.

Provide Evidence-Based Behavioral Health Interventions

  • Deliver short-term therapeutic support for patients experiencing anxiety, depression, and related behavioral health conditions.
  • Utilize evidence-informed interventions, including:
  • Cognitive Behavioral Therapy (CBT)
  • Behavioral Activation
  • Goal-focused interventions
  • Mindfulness-based strategies

Collaborate with an Integrated Care Team

  • Partner with primary care providers, psychiatrists, and behavioral health professionals to coordinate patient care.
  • Participate in treatment planning and communicate clinical updates and recommendations.
  • Identify changes in patient status and recommend appropriate adjustments to care plans.

Educate and Support Patients

  • Provide psychoeducation on behavioral health conditions and treatment options.
  • Teach practical coping strategies that promote long-term wellness and self-management.
  • Connect patients and families with educational materials and community resources.

Monitor Progress and Outcomes

  • Maintain ongoing contact with patients to evaluate treatment progress.
  • Utilize validated measurement-based care tools such as
    PHQ-9and
    GAD-7.
  • Modify interventions based on symptom improvement and clinical response.

Documentation & Compliance

  • Complete timely and accurate documentation of assessments, interventions, treatment plans, and patient communications within
    Epic
    .
  • Maintain compliance with HIPAA, organizational policies, and professional documentation standards.

Care Coordination & Community Resources

  • Assist patients in overcoming barriers to behavioral health services.
  • Coordinate referrals to specialty providers, community organizations, and support services.
  • Advocate for equitable access to behavioral healthcare.

Qualifications

Required

  • Licensed Master Social Worker (LMSW)in New York State
  • Fluent in Spanish and English
  • Experience providing behavioral health assessments and therapeutic interventions
  • Strong communication and collaboration skills
  • Experience documenting in an Electronic Health Record (EHR), preferably
    Epic

Preferred

  • At least1 year of experience working with pediatric patients
  • Experience using
    PHQ-9,
    GAD-7, or other standardized behavioral health screening tools
  • Experience working within a collaborative or integrated care model

Schedule

  • Full-Time
  • Monday–Friday
  • 9:00 AM – 5:00 PM
  • 100% Onsite
