Role Description This is a full-time, on-site role based in Nassau County, NY for a Home Health Aide and Personal Care Aide. The role involves assisting clients with activities of daily living such as bathing, dressing, grooming, toileting, and mobility support. Responsibilities include helping with meal preparation, light housekeeping, and maintaining a safe, clean home environment. The aide provides companionship, observes and reports changes in client condition, and follows care plans and agency policies.
The role may also involve supporting medication reminders, coordinating with nursing staff, and participating in ongoing training and compliance procedures to uphold quality standards.
Qualifications
- Current Home Health Aide (HHA) or Personal Care Aide (PCA) certification in the state of New York, or eligibility to obtain required certification.
- Demonstrated ability to assist with personal care tasks, including bathing, dressing, grooming, toileting, and safe patient transfers.
- Experience with basic household support such as meal preparation, light housekeeping, and maintaining a safe living environment.
- Strong interpersonal skills with the ability to provide respectful companionship, communicate clearly, and build trust with clients and families.
- Reliability in following care plans, documenting services, and reporting changes in client condition to supervisors or nursing staff.
- Ability to work on-site in Nassau County, NY, including travel to client homes as assigned; consistent punctuality and time management.
- Understanding of infection control, safety protocols, and regulatory standards in home care settings; willingness to participate in ongoing training.
- High school diploma or equivalent preferred; prior experience in home care, community-based services, or a healthcare setting is beneficial.
- Capacity to perform the physical aspects of the role, including standing, walking, and safely assisting with mobility and transfers.
