Requisition Comments and Notes: CRU MUST START IN THE NEXT WEEK AND HAVE LICENSE ON FILE AT TIME OF SUBMISSION and 7 yr background check (NY state check, nationwide, county court searches, FACIS3, and sex offender) within 18 months of start date FACIS 3 & NY state check within 30 days of start Rates are tiered based on mileage - measured by direct line not driving distance.

Travel (101 miles), Radius (51-100 miles), Local (50 miles or less) CNA for LTC: A minimum of every other weekend and/or schedule adjusted to meet the needs of the unit and patient care, but no call. Will work 80H biweekly, with weekend rotation and Mon-Sun work week may be 32H one week and 48H the next.

Requirements: AHA BLS required, please attach any others A minimum of 1 year experience is required Must have LTC experience NY license, or proof of application with applied date, required in packet at time of submission - please note NY licensing takes 6-8 weeks and we will take that into consideration with start dates/offers
