Required Documents.

1. U.S. Passport, Permanent Resident Card, or Valid Work Visa.
o If expired, please bring the extension letter or renewal documentation.
2. Social Security Card.
3. State-Issued Photo .
4. HHA or PCA Certificate.
5. Two (2) Professional Reference Letters.
o Must be written in English only.
6. Complete Physical Exam (Required).
o Must be no older than one (1) year.
o Must be fully completed, signed, and stamped by the provider.
o Include the following lab results:
Rubella and Rubeola (Measles) Titers.
Drug Screen.
PPD (Tuberculosis Test) – must be no older than 3 months.
If the PPD result is positive, a Chest X-Ray is required.
