About This Role

The Clinical Coordinator is responsible for the complex, multi-disciplinary coordination of specialty patient populations, developing, evaluating, and revising care pathways based on assessments conducted through engagement with patients, families, and multiple departments.

What You'll Do
  • Coordinate clinical care within a designated specialty with Nursing, Social Work, Care Management, and other related disciplines.
  • Establish and implement short and long-term goals, objectives, policies, and operating procedures.
  • Organize and lead day-to-day patient care coordination.
  • Provide consultation and education to interdisciplinary team members.
  • Promote an interdisciplinary approach to patient care delivery.
  • Serve as a resource for patients and their families.
  • Act as a patient advocate utilizing the Patient Bill of Rights.
  • Participate in promoting and maintaining a safe and therapeutic environment.
  • Collaborate in the development and implementation of nursing standards.
  • Maintain leadership competency through continuing education and professional organizations.
Requirements

Education:

  • Bachelor's degree in Nursing required, Master's degree in health-related field preferred.

Experience:

  • 2 years experience in specialty clinical field required.

Licenses &

Certifications:

  • Current NYS nurse licensure required.
  • BCLS certification required (Issuing Authority: AHA).
