Center Well Home Health is seeking a Home Health Occupational Therapist to help patients regain independence in their homes, through rehabilitation and functional training. You will assess daily living skills, plan care with physicians, and coordinate with a multidisciplinary team.
This role requires current OT licensure, CPR, and a valid driver’s license with reliable transportation. Preference for at least six months in home health; full-time position with competitive pay and benefits.
