The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. The Bureau of Queens Neighborhood will contribute to the larger Neighborhood Health Strategy and complement the work of the other Bureaus of Neighborhood Health in Brooklyn, Harlem, and the Bronx.
The Bureau of Queens Neighborhood health seeks to hire Program Coordinator to join the newly established Bureau of Queens Neighborhood Health.
DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:- Coordinate chronic disease programming for the Bureau of Queens Neighborhood Health, including program organization
- Research evidence based programming related to diabetes, hypertension, and asthma, including in partnership with subject matter experts and peers within the Health Department
- Maintain a working knowledge of current literature and best practices on community-based and place-based chronic disease-related interventions
- Manage participant data safely and securely under NYC Health Department guidelines and in partnership with bureau research and evaluation team
- Document and manage partner data in alignment with bureau protocols, including through the use of Public Health Partners Connect, an internal data management system
- Participate in evaluation activities as in support of programming implementation including IRB and related materials
- Support writing of grant applications and other funding proposals in support of programming
- Maintain communication with local organizations and stakeholders to promote NYC Health Department programs and resources and to collect feedback on community needs
- Engage with community partners to promote health services, resources, and education
- Represent the bureau and the Jamaica Neighborhood Health Action Center at health fairs, meetings, and other external events occurring throughout the community, as needed
- Support Bureau-wide programs and initiatives;
Participate in Just in Time training with NYC Health Department divisions that serve as a lead for a local/hyperlocal emergency response - Support agency e forts in building resilience in identified communities to mitigate impact from localized emergencies such as legionella outbreaks; conduct messaging, communicating and other efforts to make community aware of climate health preventive measures.
SKILLS:
- Two (2) plus years of community work experience
- Excellent organizational skills and highly detail oriented;
- Excellent oral and written communication skills;
- Demonstrated ability to effectively establish and maintain working relationships with culturally diverse community groups;
- Knowledge of Queens local communities and local organization leadership, specifically Jamaica and surrounding areas;
- Comfortable speaking in public;
- Experience and comfort applying a racial equity lens;
- Bilingual (written and spoken) Spanish, Bengali, Mandarin, or other language proficiency encouraged to apply
