Catholic Charities Brooklyn and Queens, Inc. is seeking a Care Coordinator to support individuals with complex medical and behavioral health needs in Brooklyn and Queens.
You will coordinate medical, social, housing resources, and ensure timely follow-up within a collaborative care team. Requirements include a bachelor’s degree in social work or related field, strong communication and crisis intervention skills, and proficiency with MS Office;
Spanish, Russian, Creole, or Cantonese language
