Job Summary

Clinical Assistant/ Integrated Care Coordinator

Position summary: The ICC Clinical Assistant Coordinator supports daily clinical and operational workflows across multiple specialty service lines and ICC programs by assisting providers with in‑house testing, specialty procedures, and day‑to‑day patient care support. This position ensures patients receive compassionate, timely, and well‑organized care by focusing on direct clinical assistance, procedural support, patient flow, and basic coordination needs of complex chronic care management cases.

Responsibilities
  • Perform patient intake and triage, including confirming identity, obtaining vitals, and updating history, medications, and allergies in the EHR.
  • Greet and escort patients to triage/exam rooms in a timely manner.
  • Assist providers with in‑house procedures and specialty testing (e.g., EKGs, PFTs, gynecologic exams) and prepare patients and rooms for visits.
  • Maintain clean, organized, and fully stocked exam and procedure areas; sterilize instruments and follow infection control protocols.
  • Draw, label, log, reconcile, and send labs; complete accurate, timely clinical documentation and coordination logs.
  • Monitor patient flow and wait times, communicate delays, and support same‑day schedule optimization, including confirmations, no‑show outreach, and filling gaps.
  • Schedule follow‑up appointments, assist with basic transportation needs, and educate patients on available resources as directed.
  • Serve as a routine point of contact for patients and families and respond to assigned messages and tasks from providers and leadership.
  • Support front desk functions during high‑volume periods (check‑in, check‑out, follow‑up booking).
  • Escalate complex medical, behavioral, or social needs to appropriate care management or clinical leadership.
  • Complete opening and closing duties (room readiness, temperature logs, cleaning, restocking, equipment shutdown, PHI security) and report low stock or site issues promptly.
  • Develop, update, and maintain individualized care plans in collaboration with patients, providers, caregivers, and the care team.
  • Support patients after emergency room visits, hospital admissions, inpatient discharges, or other transitions of care.
  • Connect patients to community resources that support health, safety, housing, food access, transportation, benefits, and overall well‑being.
Qualifications

Required Qualifications:

  • Certification as a Medical Assistant (CMA or RMA) preferred.
  • Associate’s Degree
  • Strong understanding of infection control and clinical protocols.
  • Experience supporting clinical workflows, patient triage, and in‑office procedures in a medical office or ambulatory setting preferred.
  • Excellent interpersonal, communication, and organizational skills.
  • Ability to work collaboratively across clinical and administrative teams.

Required Skills:

  • Competency in conducting triage, assisting with medical procedures, and performing sterilization and autoclaving.
  • Excellent communication skills to interact effectively with patients and healthcare providers.
