Bilingual Social Care Navigator

Position Summary Queens Childcare Network (QCCN), a program of Queens Community House, seeks a compassionate, organized, and bilingual (English/Spanish) Social Care Navigator (SCN).The Social Care Navigator will work directly with Medicaid members to identify Health
- Related Social Needs (HRSNs), develop person-centered care plans, coordinate referrals, and provide Enhanced Care Management (ECM) navigation services that improve health outcomes and family stability.
This position places a strong emphasis on connecting pregnant individuals, postpartum families, infants, and young children to community resources, including childcare, WIC, healthcare, nutrition assistance, housing, transportation, behavioral health, and other essential services. The SCN will accurately document all activities within the designated care management platforms while ensuring high-quality, culturally responsive, and trauma-informed services.
Key Responsibilities Social Care Navigation Conduct Health-Related Social Needs (HRSN) screenings with Medicaid members.
Complete comprehensive assessments and develop individualized care plans.
Coordinate Enhanced Care Management services consistent with NYS Social Care Network requirements.
Provide ongoing care coordination through regular follow-up with participants.
Assist families in overcoming barriers to accessing healthcare and community services.
Monitor progress toward care plan goals and document outcomes.
Childcare & Family Navigation Assist eligible families in accessing subsidized and publicly funded childcare programs.
Guide families through childcare eligibility, application, enrollment, and recertification processes.
Coordinate with Family Child Care providers, Early Childhood Programs, and community partners.
Support families in maintaining childcare stability while addressing barriers that may impact employment, education, or healthcare access.
Resource Coordination Provide navigation and referrals for:
WICSNAPMedicaid
Housing resources
Food assistance
Transportation
Behavioral health
Maternal and infant health services
Pediatric care Developmental screenings
Early Intervention Home visiting programs
Community-based supports
Community Outreach Participate in outreach events throughout Queens.
Conduct community presentations and enrollment events.
Build relationships with healthcare providers, community organizations, and partner agencies.
Promote QCCN's Social Care Navigation services to eligible families.
Documentation & Compliance Maintain accurate and timely documentation within the SCN care management platform.
Document screenings, care plans, referrals, follow-up activities, and outcomes.
Ensure compliance with SIPPS Social Care Network, NYS DOH, Medicaid, HIPAA, and organizational requirements.
Meet productivity, quality assurance, and documentation standards.
Skills & Competencies Willingness and ability to travel within the QCCN catchment area.
Strong time management, organization, and documentation skills.
Ability to multitask and prioritize responsibilities in a dynamic environment.
Demonstrated collaborative mindset and ability to support peers in a leadership capacity.
Deep respect for and understanding of underserved communities.
Strength-based communication, coaching, and relationship-building skills.
Bachelor's degree in Social Work, Human Services, Public Health, Psychology, Early Childhood, or related field; equivalent experience may be considered.
Ability to fluently read, write, and speak in English and Spanish.
Minimum two years of experience working with families in health care, social services, early childhood, care coordination, case management, or community- based programs.
Knowledge of Medicaid and community resources.
Strong organizational and documentation skills.
Excellent interpersonal and communication skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Proficiency with Microsoft Office and electronic documentation systems.
