PRN Speech‑Language Pathologist (SLP)

The PRN SLP will provide high‑quality speech, language, cognitive, and swallowing therapy services to residents in a skilled nursing setting. This role requires adaptability, strong clinical judgment, and collaboration with an interdisciplinary team to deliver exceptional patient care.

• Evaluate, treat, and document speech, language, cognitive, and swallowing disorders in accordance with physician orders and plan of care.
• Develop and implement individualized treatment plans.
• Maintain accurate, timely documentation in compliance with regulatory standards.
• Collaborate with nursing, therapy team members, physicians, and families.
• Participate in care planning and interdisciplinary team meetings.
• Screen residents for communication problems and conduct in‑depth evaluations to make written recommendations for a therapy plan of care.
• Coordinate audiometric screening tests and refer for further audiology evaluations or treatments if necessary.
• Initiate speech‑language therapy plan of care according to physician orders and maintain required contact with the physician during the therapy program.
• Observe residents’ rights and treat residents with respect and kindness.
• Maintain written evaluations, progress notes, and discharge summaries for all residents in a speech‑language treatment program.
• Confere with other staff regarding special needs of residents and integrate the speech‑language treatment program with goals and approaches of other disciplines.
• Attend resident care planning and family/resident care plan review meetings for residents currently in a speech‑language treatment program.
• Maintain records including statistical reports about residents, in‑service education information, budget information, and material/equipment inventory.
• Submit statistical reports to the Business Office staff for Medicare billing payments and complete assessment forms upon request.
• Attend staff meetings and required or preferred in‑service training.
• Participate in developing and conducting in‑service training programs for facility personnel involved in resident care.
• Review written speech‑language policies and procedures annually and make necessary revisions.
• Maintain costs of the program within the parameters of the annual budget.

• Current New York State license as a Speech‑Language Pathologist.
• M.A. or M.S. Degree in Speech‑Language Pathology from an approved school required.
• Certificate of Clinical Competence (CCC‑SLP) preferred;
  Clinical Competence Certification from ASHA preferred.
• Experience in a skilled nursing facility preferred.
• Strong clinical, organizational, and communication skills.
• Flexibility to work as needed based on facility demands.
• Ability to work independently and as part of a team.
• Current, valid CPR/BLS credential.
• Ability to communicate well verbally and in writing required.
• Ability to read, write, speak, and understand English at an intermediate or more advanced level.
• Verification of qualifications by the State Department of Health.
• Rehabilitation Therapies license number/certification number required.
