Description

Ohel is seeking Peer Specialist to be a part of a field‑based team that serves participants transitioning from the hospital back into the community. The role will involve utilizing one's lived experience to bridge the care gap by collaborating with hospital staff to facilitate and maintain the transition from the hospital back into the community. A peer will address the preparatory needs of participants in the early stages of recovery, assist in the development of daily living skills, and coping mechanisms through group and individual work in the community.

While working with the client a peer would provide care navigation and fosters connection and engagement with community‑based organizations that promote a sense of purpose, physical wellness, education, employment, socialization, and community involvement.

Responsibilities
  • Engage participants in transition planning and needs assessment
  • Conduct individual sessions focusing on independent living and coping skills
  • Provide emotional and practical support during community transition
  • Foster and maintain collaborative communication with community providers and collaterals
  • Advocate for participants' personal goals and address barrier to obtaining those goals
  • Maintain comprehensive documentation following contact with clients and collaterals
Qualifications
  • High School Diploma or equivalent
  • New York State Certified Peer Specialist required
  • Experience in advocacy and behavioral health services preferred
  • Must have lived experience with mental health
  • Ability to use personal story for empowerment and recovery

The ideal candidate is a self‑starter who can work independently as well as within a team and has excellent follow up skills. This full‑time position is located in Brooklyn. Hybrid schedule offered, with inpatient, home and community visits required. Position includes a competitive benefits package.

Salary: $45,000
