** Shift Details:*
* + Monday-Friday 8:00am-4:30pm

** Social Worker

Duties & Responsibilities:

*
* + Assessing individuals, developing individual care plans and determining necessary resources.

+ Advocating for and assisting individuals and families in accessing and obtaining services.

+ Contacting service providers and attending interdisciplinary conferences to determine appropriate placement and referrals.

+ Monitoring individual rehabilitation goals and progress.

+ Providing crisis intervention services.

** Minimum Qualifications *
* + Current, valid, and unrestricted NY license/certification in good standing, as required for the position.

+ Minimum of one (1) year related experience

+ Bachelor's in Social Work degree, working under the supervision of a licensed master social worker (LMSW) or licensed clinical social worker or New York State LCSW or LCSWs.

+  Valid drivers license.

+ Additional certifications may be required.

+ Copy of current, completed

Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

+ Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
