Social Worker
Provide specialized social work services to patients, including assessment, counseling, and care planning. Support community outreach, advocate for patients, and collaborate with healthcare providers to ensure comprehensive, culturally competent care.
Responsibilities:
- Assess the psychosocial needs of individuals, families, and partners to develop appropriate intervention plans.
- Conduct social work evaluations and deliver ongoing social services tailored to patient needs.
- Provide individual, family, and group counseling while coordinating referrals for psychiatric evaluation or treatment.
- Develop and monitor comprehensive care plans, evaluating progress regularly with clients.
- Facilitate linkage between patients, healthcare providers, and social service agencies to promote continuity of care.
- Participate in community outreach, education programs, staff meetings, and case conferences to ensure coordinated service delivery.
Qualifications:
- Hold a Bachelor's Degree in a health-related field such as Social Work or Community Health.
- Demonstrate experience in HIV or substance abuse counseling preferred.
- Bilingual capabilities preferred to serve diverse communities effectively.
- Possess strong assessment and counseling skills with a commitment to culturally competent service.
- Be familiar with community resources and healthcare systems involved in social work.
- Exhibit excellent communication and advocacy skills to support patient access and retention.
