Scope of Position:
The Clinical Coordinator plays a vital role in supporting residents in supportive housing by assessing and responding to clinical needs that may impact their ability to maintain stability, independence, and overall well-being. As part of The Bridge’s comprehensive behavioral health team, the Clinical Coordinator provides clinical guidance, coordinates appropriate services and interventions, and collaborates with residents, staff, and community providers to address mental health and substance use concerns.
This role helps ensure that residents receive individualized, evidence-based support that promotes recovery, stability, and meaningful engagement in their communities.
Essential Position Functions:
Provide direct support and/or linkage to referrals for a small caseload of residents living in Bridge housing throughout the Bronx and Manhattan.
Provide clinical direction and guidance to supportive housing staff.
Collaborate with internal and external stakeholders to promote increased coordination of care.
Utilize the agency’s electronic health record system to document services and track data.
Promote residents’ physical and mental health, safety, well-being, and recovery.
Master’s degree or higher in Social Work or Mental Health Counseling.
Experience working with individuals living in supportive housing and/or with mental health diagnoses is preferred.
Experience supporting individuals living in supportive housing with complex needs.
Computer literacy required.
Ability to utilize the agency’s Electronic Health Record (EHR) system to document services.
Bilingual Spanish-speaking skills are a plus.
