Centers Assisted Living Program is seeking Certified Home Health Aides to provide compassionate, hands-on care to seniors and adults in a supportive, home-like setting. CHHAs assist residents with bathing, dressing, grooming, toileting, mobility, and meals while promoting dignity, safety, and independence. Responsibilities include vital sign monitoring, medication reminders, documenting care, and promptly reporting changes in condition. You'll support social and recreational activities, maintain a clean, safe environment, and collaborate closely with nurses and other team members to deliver resident-centered, regulatory-compliant care every shift.
Responsibilities
- Provide hands-on assistance with bathing, dressing, grooming, toileting, and personal hygiene
- Support safe transfers, ambulation, and use of mobility devices
- Assist with meal setup, feeding, and monitoring intake as directed
- Take and record vital signs; observe and report changes in residents' conditions
- Offer medication reminders as directed by nursing staff and care plans
- Document care provided in accordance with facility and regulatory standards
- Maintain a clean, safe, and home-like environment for residents
- Support resident social and recreational activities to promote engagement
- Follow infection control, safety, and emergency procedures at all times
- Collaborate with nurses and interdisciplinary team to deliver resident-centered care
Required Skills
