Description- FNP/ANP
- Ages seen based on what the provider is certified to see
- Provider retiring at the end of the month
- Will accept a provider willing to do a minimum of 3 days/week
- Basic primary care visits, sick/well, check-ups etc.
- Approx 10-15 patients per day
- Support Staff: 1:1 MA to assist
- EMR: ECW
- No Call
- Provider MUST HAVE Medicare/Medicaid
- NY License
- Medicare/Medicaid
