Patient Coordinator

Access Physical Therapy & Wellness, a leading outpatient rehabilitation provider, is seeking a Patient Coordinator for our Hopewell Junction clinic. In this front-desk role, you will welcome patients, manage check-in/check-out, schedule visits for physical, occupational, and speech therapy, and verify insurance and authorizations. You will maintain accurate records in our EHR, handle calls and referrals, and support a smooth daily workflow.

Responsibilities

• Greet and check in/out patients in a courteous, professional manner
• Schedule and confirm patient appointments across therapy disciplines
• Verify insurance eligibility and collect co-pays and required documentation
• Maintain accurate patient records in the EHR system
• Coordinate communication between patients, therapists, and referring providers
• Answer phones, respond to inquiries, and route messages promptly
• Ensure compliance with HIPAA and clinic policies
• Support daily clinic operations, including paperwork and scanning
• Assist with tracking authorizations and referrals
• Contribute to a collaborative, patient-centered clinic environment
