Patient Care Coordinator in Brooklyn EXPERIENCED

Summit Home Care is seeking Patient Care Coordinators who are experienced as Patient Care Coordinators and who speaks FLUENT English and Spanish for our Brooklyn office.

Patient Care Coordinators are responsible for matching patients with the right home health aides, providing basic case management services and are responsible for maintaining communication with patients, office personnel, paraprofessional staff, and contract agencies.

Starting salary $24 - $27 per hour (depending on experience)

• Experienced patient care coordinator with a minimum of at least one year of experience in home care coordination.
• Must speak FLUENT English and Spanish. Really, if you don't speak both FLUENT English and Spanish, please do not apply.
• HHA Exchange experience is required.
• Excellent telephone and communication skills
• Excellent organizational skills
• Ability to maintain accurate schedules, notes and records
• Good computer skills
• Good interpersonal skills
