Home Health Aide (HHA) – Resident Transport

The Phoenix Rehabilitation and Nursing Center is seeking a reliable and compassionate Home Health Aide (HHA) to assist with the safe transport of residents within and outside the facility. This is a vital role that ensures residents arrive safely to appointments, activities, and therapy sessions while providing friendly and supportive care.

This position is primarily focused on resident transportation
, making it ideal for candidates who enjoy working directly with seniors and supporting their mobility and well-being.

• Safely transport residents to and from medical appointments, therapy sessions, and facility activities

• Provide assistance and support to residents during transfers and mobility challenges

• Monitor and ensure resident safety and comfort during transport

• Communicate effectively with the care team regarding resident needs and observations

• Assist with light resident care and companionship as needed

• Follow all facility policies, procedures, and safety guidelines

• HHA certification in good standing

• Valid driver’s license and clean driving record (if transporting offsite)

• Ability to lift and assist residents safely

• Strong communication and interpersonal skills

• Compassionate, patient, and resident-focused

• Prior experience in a skilled nursing or rehabilitation setting preferred but not required

• $19 per hour
