We're seeking a compassionate and skilled Home Health Aide (HHA)/PCA to join our team and provide exceptional care to our clients in the East Bronx. As a Home Health Aide (HHA)/PCA, you'll work closely with our team to deliver high-quality care and support to patients in their homes, emphasizing patient-centered care and cultural sensitivity.

• Provide personal care assistance, including bathing, dressing, and grooming, as needed
• Assist with daily living activities, such as feeding, toileting, and incontinence care
• Support mobility and ambulation, including transfers and walking
• Administer medication as ordered by a healthcare professional
• Assist with meal preparation, feeding, and hygiene
• Maintain accurate and detailed records of patient care
• Collaborate with healthcare professionals to develop and implement individualized care plans
• Provide emotional and psychological support to patients and families

• Current certification as a Home Health Aide (HHA) or Personal Care Assistant (PCA)
• High school diploma or equivalent required
• Ability to work in a fast-paced environment with minimal supervision
• Strong communication and interpersonal skills
• Compassionate and empathetic attitude towards patients and families
