Location: New York
Well Life Network is seeking an ACT Resource Manager to support programs by helping recipients obtain and maintain essential benefits. You will monitor eligibility, coordinate applications, and collaborate with ACT teams to align resources with treatment goals.
You will educate recipients, document in the EMR, and provide crisis intervention as needed, while traveling to ACT sites and supporting families and individuals in accessing public resources.
