Role:
Licensed Physical Therapist providing one-on-one care to older adults in private homes and senior living communities, utilizing the Geriatric House Calls™ model.
Responsibilities- Conduct evaluations, develop care plans, and deliver evidence-based treatments to support mobility, independence, and wellness.
- Document care within EMR systems and collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff on safety and aging-in-place strategies.
- Incorporate telehealth and virtual care to enhance access and continuity.
- Valid PT license and degree from an accredited program.
- Proficiency in computer use and EMR systems, current CPR certification.
- Ability to meet physical demands such as lifting, repositioning patients, and performing therapeutic activities.
Provide personalized, in-home therapy, develop tailored treatment plans, support patient independence, and work within a flexible, interdisciplinary environment to improve patient outcomes.
