Compensation Range: $65,000 - $70,000 yearly commensurate with experience

Overview:

The Program Manager for outreach and prevention is responsible for the day-to-day coordination and oversight of outreach, HIV prevention, testing, linkage/navigation, and engagement services within the Youth & Prevention Services portfolio. In addition, this position directly oversees two contracts: the CDC-funded CON condom distribution program and the AIDS Institute COC-funded MPACT program. This position supports high-quality, culturally responsive services for young people, LGBTQ+ individuals, communities of color, and other priority populations impacted by HIV, housing instability, substance use, mental health needs, vocational needs and other social and structural determinants of health.

The Program Manager will supervise the Outreach Manager and assigned outreach and navigation staff, monitor service delivery and documentation, support implementation of HIV/STI/HCV testing and prevention protocols, coordinate community and digital outreach strategies, and ensure timely warm handoffs to clinical, behavioral health, substance use, PrEP, and supportive services. The role works closely with YPS and Prevention and Care leadership and cross-department partners to strengthen program operations, contract performance, quality improvement, and participant experience.

Responsibilities:

  • Provide day-to-day supervision, coaching, scheduling, and support to assigned staff to ensure consistent program coverage and high-quality service delivery.
  • Coordinate outreach and engagement strategies, including community-based outreach, alternative venue outreach, drop‑in center engagement, social media/digital engagement, presentations, and partnership-based referrals.
  • Oversee HIV prevention services workflows, including HIV/STI/HCV testing, PrEP education and referrals, prevention counseling, linkage to care, and follow‑up navigation.
  • Support implementation of testing and prevention protocols, including informed consent, specimen collection, test administration, result delivery, referral documentation, and warm handoffs, in alignment with city, state, funder, and agency requirements.
  • Monitor participant flow, outreach outcomes, testing activity, linkage/navigation follow‑up, enrollment and discharge trends, and other service indicators to support retention, quality, and contract performance.
  • Review documentation for accuracy, timeliness, and completeness, including assessments, navigation notes, service plans, referrals, encounter documentation, follow‑up attempts, and other required records.
  • Coordinate with Housing Works clinical care, behavioral health, substance use treatment, Health Home/care coordination, and supportive service teams to address participant barriers and improve engagement in care.
  • Develop and maintain relationships with community partners, schools, providers, shelters, CBOs, LGBTQ+ youth‑serving organizations, and other stakeholders to expand access to Housing Works’ services.
  • Support program quality assurance and continuous quality improvement activities, including chart reviews, data audits, service trend monitoring, corrective action follow‑up, and preparation for internal or funder reviews.
