Pomelo Care seeks a contract Registered Dietitian to support midlife women, pregnant people, and young families through virtual, evidence-based nutrition care. You will conduct telehealth assessments, develop individualized nutrition plans, and coordinate closely with Pomelo’s multidisciplinary care team to improve maternal, newborn, and infant outcomes. Responsibilities include ongoing follow-up, motivational interviewing, and documentation in our digital platforms. You will use data and clinical guidelines to inform care, contribute feedback to enhance programs, and support Pomelo’s mission in a flexible, remote-first environment.
Responsibilities- Provide virtual nutrition assessments for midlife, pregnant, and postpartum patients and families
- Develop individualized, evidence-based nutrition care plans aligned with clinical guidelines
- Deliver ongoing telehealth counseling using motivational interviewing and behavior-change techniques
- Collaborate with physicians, nurses, and care coordinators to support maternal and infant health outcomes
- Document visits and care plans accurately in electronic systems and digital tools
- Monitor patient progress, adjust plans, and elevate concerns to the care team as needed
- Use data and quality metrics to inform care decisions and improve outcomes
- Participate in team meetings, case reviews, and program improvement discussions
- Educate patients on chronic disease prevention, weight management, and infant nutrition
- Uphold privacy, ethical, and regulatory standards in virtual care delivery
