Per Diem Occupational Therapist; OT

@Home Therapy is expanding and looking for a licensed Occupational Therapist to provide home health services to adult and geriatric patients in the Elmhurst community.

We are seeking a professional who values patient-centered care and enjoys helping individuals improve their functional abilities and quality of life.

• Conduct occupational therapy assessments and evaluations.
• Identify barriers to safe and independent daily living.
• Design treatment plans based on each patient's individual needs.
• Provide training on adaptive equipment and energy conservation.
• Educate patients and families on fall prevention and home safety.
• Track patient outcomes and adjust treatment when appropriate.
• Coordinate with physicians and other healthcare professionals.

• Flexible availability.

• Independent contractor role.
• One-on-one home-based care.
• Support from our scheduling and administrative team.

• A valid New York State OT license.
• Strong interpersonal and clinical reasoning skills.
• The ability to manage patients independently.
• A commitment to quality patient care.

Previous home care experience is helpful, but we welcome qualified therapists from other clinical backgrounds as well.
