Location: New York
New York-Presbyterian Hospital is seeking an Assistant Discharge Planner for the Hudson Valley region. The role is full-time, day shift, based at 1980 Crompond Road, Cortlandt Manor, NY, within the hospital’s care management ecosystem to improve patient flow and discharge accuracy.
You will work with Care Management and Social Work to ensure timely discharges, with rounds and detailed communication to the multidisciplinary team, supporting patient-centered care and efficient LOS management.
