MCMH is seeking a Full-Time Behavioral Health Clinician with CASAC credentials to join our integrated clinical team.

The clinician's role is to deliver comprehensive, person-centered care that addresses substance use and mental health needs (when present), consistent with the NYS Certified Community Behavioral Health Clinic (CCBHC) model
. The role serves as an integral member of the multidisciplinary team, participating in and/or leading the development and implementation of clinically sound and culturally relevant counseling and/or psychotherapy services and often collaborating with peers, care managers and other professionals to ensure whole person care. The clinician provides services in a compassionate, recovery-oriented, and person-centered manner, consistent with MCMH principles. The clinician will actively support individuals in achieving their goals, resilience, and increased self-sufficiency as central goals of treatment.

Under supervision, the Behavioral Health Clinician participates in and/or performs functions related to counseling, diagnostics, treatment planning, rehabilitation, assessment, intervention, and other supportive services for patients in treatment.

  • Qualified behavioral health professional – Advanced or Master CASAC is required. Additional licensure is desired but not required (LCSW, LMSW, LMHC, LMFT, or Licensed Psychologist).
  • Previous clinical experience (two years or more) providing direct psychotherapy/counseling services to adults, children, adolescents, and families. Demonstrated experience in a setting that provides both mental health and substance use treatment is highly preferred.
  • Knowledge of and commitment to the NYS Certified Community Behavioral Health Clinic (CCBHC) model and its core principles, including whole-person care and team-based delivery.
  • Proficiency with computer, information systems, and Electronic Health Records.
  • Ability to communicate with all levels across the organization effectively.
  • Comfort working in a community-based setting with a diverse patient population.
  • Strong organizational, verbal communication, and writing skills.
  • Bilingual (Spanish/English) is preferred.
Responsibilities
  • Provide individual counseling/ therapy sessions to assigned patients in alignment with individualized person-centered treatment plans.
  • Facilitate family counseling sessions, when clinically appropriate, to support the well-being of the patient within the context of their family system.
  • Facilitate evidence-based group therapy sessions, utilizing clinically appropriate interventions that support treatment goals.
  • Conduct comprehensive psychosocial assessments, including psychosocial histories and completion of evidence-based screening tools, to formulate diagnostic impressions and develop a clinically sound formulation (licensure and scope of practice permitting).
  • Make decisions about, and maintain required records of, appropriate Level of Care using patient criteria tools (NYS-LOCADTR) as needed.
  • Develop and continuously monitor treatment plans in collaboration with patients and members of the multidisciplinary team, including establishing treatment goals, objectives, and specific interventions. Accurately record treatment plans consistent with established CCBHC regulations and applicable administrative protocols.
  • Conduct extensive outreach for re-engagement efforts when patients miss weekly individual, group, or other scheduled clinical appointments.
  • Participate in the evaluation of clinical progress and monitoring of patients’ treatment plans.
  • Monitor Medication Management under the guidance of a licensed prescriber with close clinical follow-up.
  • Maintain contemporaneous documentation in confidential case records within the EMR for services rendered (in relation to treatment goals and objectives), outreach/engagement efforts, coordination of care, and all other patient-related consultations with third-party collaterals, in the form of progress notes/patient notes.
  • Collaborate with the Quality Assurance and Admissions Departments to ensure all patient documentation is in compliance and meets regulations.
  • Actively participate as a core member of an integrated care team, collaborating with care managers, peer specialists, and other medical providers to coordinate care and ensure a seamless patient experience.
  • Provide crisis intervention for patients as needed.
  • Attend weekly individual and group supervision and contribute to clinical case conferences and team meetings.
