Environment

Our facility operates onsite, providing a welcoming environment that promotes healing and rehabilitation. As a key member of our rehabilitation team, you will work alongside skilled occupational therapists, nurse practitioners, and other healthcare providers in a facility designed for optimal patient care. Our team-oriented approach ensures that you are well-supported in your role while directly engaging with patients throughout their recovery.

Key Responsibilities
  • Conduct thorough evaluations of patients' physical abilities and rehabilitation needs, establishing personalized treatment plans based on their specific circumstances.
  • Provide one-on-one therapy sessions with up to 7 patients per shift, delivering tailored interventions aimed at enhancing strength, mobility, and overall physical function.
  • Utilize various therapeutic techniques, including manual therapy, therapeutic exercise, and modalities to address each patient's unique challenges.
  • Maintain clear and open lines of communication with patients and their families, offering education on care plans and progress updates to encourage engagement and understanding.
  • Document all patient interactions, treatment plans, and progress notes accurately and within required time frames to ensure compliance and continuity of care.
  • Collaborate with other healthcare professionals to develop multidisciplinary approaches, participating in care team meetings to discuss patient progress and care adjustments.
Patient Population / Client Focus

You will primarily work with a diverse patient population, including individuals recovering from orthopedic surgeries, neurological conditions, and musculoskeletal injuries. Our facility focuses on providing rehabilitation services to both seniors in skilled nursing and younger adults who require intensive physical therapy post-surgery or after experiencing traumatic injuries.

Qualifications
  • Active Physical Therapist License in the state of New York.
  • Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
  • A minimum of 2 years of hands-on experience in a rehabilitation or skilled nursing facility, demonstrating a strong background in patient-centered care.
Preferred Experience
  • Experience with geriatric populations and complex care management.
  • Familiarity with electronic health record (EHR) systems for accurate documentation.
  • Certification in specialized therapy techniques (e.g., vestibular therapy, aquatic therapy) is a plus.
Compensation and Benefits

We offer a competitive salary range of $100,000 to $120,000 per year, dependent on experience and qualifications.
