Evolve Therapy Services is seeking a compassionate Physical Therapist to join our in-house Skilled Nursing Facility in Oakdale, NY. You will evaluate residents, develop treatment plans, and coordinate with an interdisciplinary team to ensure quality resident care.
Responsibilities include screening, documenting per CMS requirements in Net Health EMR, and delivering therapy services in accordance with standards and productivity guidelines. Weekend PRN schedule offered with opportunities for growth.
