Overview:

Schervier Nursing and Rehab a leading provider of compassionate and high-quality care to our elderly residents, is seeking a dedicated Social Worker to join our team. This role is ideal for a professional who is passionate about supporting the mental, emotional, and social well-being of nursing home residents and their families.

Responsibilities:
  • Conduct psychosocial assessments to identify the needs of residents and their families.
  • Provide counseling and emotional support to residents and families regarding adjustment to nursing home life.
  • Develop and implement care plans in collaboration with healthcare team members.
  • Advocate for resident needs, rights, and preferences.
  • Coordinate services and resources for residents, including community resources and support groups.
  • Facilitate resident and family meetings and provide guidance on healthcare, end-of-life planning, and other related topics.
  • Document and maintain accurate records of all interactions and services provided.
  • Assist in discharge planning and post-discharge follow-up care.
Qualifications:
  • Master's degree in Social Work (MSW) from an accredited program.
  • Active Social Worker license (LCSW or equivalent) in the state of NY.
  • Previous experience in a healthcare or nursing home setting preferred.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to work effectively with a multidisciplinary team and handle sensitive situations with compassion.
