The rehabilitation center is designed to promote healing and recovery, featuring spacious treatment areas equipped with advanced therapy tools and modalities. As we focus on improving the quality of life for our patients, our team collaborates closely, sharing insights and techniques to provide comprehensive care. This onsite role allows for direct interaction with patients, their families, and other healthcare professionals, enabling a supportive network that enhances patient outcomes.
Key Responsibilities- Evaluate patient conditions through thorough assessments, developing individualized treatment plans aimed at achieving specific rehabilitation goals.
- Administer hands-on therapeutic interventions such as manual therapy, therapeutic exercise, and modalities to promote recovery and enhance mobility.
- Monitor and document patient progress carefully, making adjustments to treatment plans as needed to ensure optimal outcomes.
- Educate patients and their families about treatment protocols, home exercise programs, and preventative measures to support long-term wellness.
- See an average of 7 patients per day, managing a busy caseload while providing personalized attention to each individual's needs.
- Collaborate with nursing staff, occupational therapists, and other health professionals to develop comprehensive care strategies.
- Provide timely and accurate information to patients and their families regarding therapies, addressing all inquiries and directing them to appropriate resources when necessary.
- Contribute to departmental meetings and ongoing professional development initiatives to enhance skills and improve service delivery.
In this role, you will work with a diverse patient population, including elderly individuals recovering from surgical procedures, those with chronic pain, and patients undergoing rehabilitation after accidents or strokes. Our focus is on creating individualized care plans that cater to the unique needs of each patient, emphasizing functional recovery, mobility restoration, and overall well-being.
Qualifications- Current New York State Physical Therapist License is required.
- A Master of Science in Physical Therapy (MSPT) or Doctor of Physical Therapy (DPT) degree is essential.
- At least 2 years of relevant experience in a clinical setting, preferably in rehabilitation.
While not mandatory, candidates with additional certifications in specialized therapeutic areas such as orthopedics or geriatrics will be viewed favorably. Previous experience in a skilled nursing facility or comprehensive rehab setting is a plus, as it aligns closely with our patient care model.
