Facility: Staten Island Care Center
Position: Social Services Worker
Location: Staten Island, NY
Shift: Full Time | 9:00 AM – 5:00 PM
Salary: $50,000 – $60,000 per year

Join Our Team!

Staten Island Care Center is seeking an experienced and compassionate Social Services Worker to support our residents and families. The ideal candidate will have strong experience in long-term care, excellent communication skills, and a passion for resident advocacy and quality care.

Responsibilities
  • Provide social, emotional, and psychosocial support to residents and families.

  • Complete assessments, social histories, progress notes, and care plans.

  • Participate in care plan meetings, MDS, discharge planning, and interdisciplinary team meetings
    .

  • Coordinate discharges, transportation, referrals, and community resources.

  • Assist residents and families with Medicare, Medicaid, financial assistance, and community services
    .

  • Review and coordinate MOLST/POLST and advance directive documentation.

  • Advocate for resident rights, preferences, and quality of life.

  • Maintain accurate documentation and ensure compliance with applicable regulations.

  • Collaborate closely with nursing, medical, rehabilitation, and other departments.

  • Participate in quality assurance activities, facility surveys, and departmental initiatives.

  • Build strong relationships with residents, families, staff, physicians, and community agencies.

Qualifications
  • 1-2 years of social services experience required.

  • Bachelor's or master's Degree required.

  • Long-term care, skilled nursing, rehabilitation, or healthcare experience preferred.

  • Knowledge of care planning, discharge planning, MDS, Medicare/Medicaid, and community resources.

  • Strong communication, organization, and documentation skills.

  • Compassionate, professional, dependable, and highly organized.

  • Strong problem-solving skills and ability to work effectively as part of an interdisciplinary team.

  • Experience with EMR systems preferred.
