ESSENTIAL RESPONSIBILITIES
- Serve as the primary Service Coordinator for assigned infants, toddlers, and families..
- Explain the Early Intervention Program, service coordination process, family rights, procedural safeguards, and available services to families..
- Communicate with families in their preferred language when qualified to do so..
- Develop trusting, respectful, and family-centered relationships with parents and caregivers..
- Assist families with navigating the Early Intervention system..
- Coordinate and facilitate communication among families, evaluators, Early Intervention providers, Early Intervention Officials/Designees, and other professionals involved with the child..
- Assist families with scheduling and coordinating multidisciplinary evaluations and other required appointments..
- Participate in and coordinate IFSP meetings as required..
- Assist families in identifying priorities, concerns, resources, and desired outcomes for their child and family..
- Support the development, implementation, review, and amendment of the child’s IFSP..
- Assist families with identifying available Early Intervention providers for authorized services..
- Coordinate the initiation of authorized Early Intervention services..
- Monitor whether services are being delivered in accordance with the child’s IFSP..
- Follow up with families and providers regarding missed, delayed, interrupted, or unavailable services..
- Identify barriers affecting service delivery and communicate concerns to the appropriate Early Intervention personnel..
- Maintain ongoing contact with assigned families in accordance with program requirements and individual family needs..
- Assist families with accessing community programs, healthcare resources, social services, educational resources, and other supports when appropriate..
- Advocate for family participation and informed decision-making throughout the Early Intervention process..
- Support transition planning as children approach age three and prepare to transition from the Early Intervention Program..
- Collaborate with families regarding potential transition to preschool special education or other appropriate community services..
- Complete all required case notes, service coordination documentation, contact logs, IFSP-related documentation, and other records accurately and within required time frames..
- Maintain accurate and current information within required Early Intervention systems..
- Maintain confidentiality of child and family information in accordance with applicable federal and New York State requirements and Angel Community Alliance policies..
- Participate in required orientation, training, supervision, quality-assurance activities, and professional-development activities..
- Follow all applicable New York State Early Intervention Program requirements, policies, procedures, and ethical standards..
- Bilingual Service Coordinators may be responsible for:.
- Communicating directly with families in both English and their qualified second language..
- Explaining Early Intervention processes and requirements in a manner families can understand..
- Supporting culturally and linguistically responsive communication..
- Helping reduce language-related barriers to participation in Early Intervention services..
- Communicating effectively with bilingual families while maintaining professional boundaries and confidentiality..
- Utilizing approved interpretation and translation resources when circumstances require services beyond the Service Coordinator’s language competency or role.. .
- Bachelor’s degree or higher in psychology, social work, human services, early childhood education, special education, sociology, counseling, family studies, behavioral science, or a related field..
