Occupational Therapist (OT) Requirements:
  • Conduct comprehensive occupational therapy evaluations to assess residents’ functional status, including ADLs, IADLs, cognition, mobility, strength, coordination, balance, and safety awareness
  • Develop, implement, and update individualized treatment plans based on physician orders, resident goals, and evidence-based practice
  • Provide skilled therapeutic interventions to improve independence in self-care, transfers, positioning, upper extremity function, and adaptive techniques
  • Address cognitive, perceptual, and psychosocial deficits impacting daily function and quality of life
  • Recommend, train, and educate residents in the use of adaptive equipment, assistive devices, and environmental modifications
  • Collaborate closely with an interdisciplinary team (PT, SLP, nursing, social services, and physicians) to coordinate care and support optimal outcomes
  • Participate in care plan meetings, discharge planning, and family education to ensure safe transitions and continuity of care
  • Monitor resident progress, modify treatment plans as needed, and ensure services meet medical necessity requirements
  • Complete timely, accurate documentation of evaluations, daily treatments, progress notes, and discharge summaries in accordance with Medicare, state, and facility guidelines
  • Maintain compliance with all regulatory, ethical, and professional standards of occupational therapy practice
  • Supervise and collaborate with COTAs and therapy aides as applicable
  • Promote resident safety, dignity, and quality of life in a skilled nursing environment
