Occupational Therapist; OT Home Health | Per Diem

@Home Therapy is looking for a compassionate and motivated New York State licensed Occupational Therapist to provide one-on-one care to adult and geriatric patients in their homes.

This opportunity is ideal for an OT who enjoys independence, flexibility, and making a meaningful difference in patients' daily lives.

• Completing in-home OT evaluations and treatment.
• Developing individualized treatment plans.
• Addressing daily living skills, mobility, coordination, cognition, and home safety.
• Educating patients and caregivers on adaptive techniques and equipment.
• Monitoring progress and updating care plans.
• Completing EMR documentation and collaborating with the care team.

• Active New York State OT license.
